I also see bursts of continuity errors since upgrading the software on my pi, and my set up is similar but not identical.
My set up is like this: the 3B+ has a TV hat, and runs tvheadend. The rootfs is on sd, but the storage for tvheadend is over NFS over wifi.
I had been running older software (last upgraded ~Autumn 2024), which was stable except the wifi would randomly lock up every couple of months - other than that, TV reception was absolutely solid. I did a dist-upgrade to pick up latest packages and the 6.12 kernel, and TV reception is now bad - with a continuity errors when streaming over wifi every few minutes. When watching TV locally with kodi, reception is completely unusable - no picture appears. (edit: this might be a separate problem)
I reverted the kernel to 6.6, but no change.
As a workaround, I have set the max CPU frequency to 600MHz:and TV reception over wifi is now stable again, although I haven't tried kodi yet.
I am also wondering if this could be the result of a firmware change, but I haven't got backups or older logs to be sure what version was there before.
