I have set it all up on another PiZ2W with another PSU, cable, SD card.
Same issues.
I've now found that there are a number of similar issues reported on the Raspberry Pi Git Hub but no concrete answer.
Seem to be an issue with Broadcom drivers somewhere some how.
I think I'll try a dongle and turn off the internal WIFI and BT.
