After further testing it seems that my CM4 gets set to whatever Wi-Fi region the access point it's connected to is configured for. I'm assuming there's some negotioation between the Pi and the access point when it first connects.
I think this is largley consistent with the behaviour seen by bls too except that I don't need to have country set in wpa_supplicant.conf.
When the Pi isn't connected to a Wi-Fi network the 'iw reg get' command reports whatever was set using raspi-config.
