That looks about right to me.
IOMMU4 (HVS) is used by Linux, but only when enabled by vc4-kms-v3d
IOMMU0,1 are not initialized at all, except perhaps to disable them. In their reset state the IOMMUs are transparent to all AXI traffic.
Even when initialized (by the current Linux driver) they remain transparent for addresses below 40GB.
They all share an "IOMMU Cache" which they need to link to in DT if enabled.
