you're on bookwork or which OS is in use?
you've enabled VNC via raspi-config?
I'm using tigervnc on my hosts which even works for displays i.e. as small as 240x135pixels. your 'issue' is most likely related to realvnc viewer and your VNC server.
why did you install that package when all you need should be there in a stock bookwork OS? never had any issues so far...
and why don't you simply rotate your screen via Screen configuration editor (or wlr-randr from CLI)?
